Member Care Pathways
Care Pathways are pre-designed sequences of activities that are automatically scheduled for members based on timing rules or conditions (like specific responses to questions).
They’re especially helpful when you want to create a structured, repeatable workflow for common member journeys—such as post-enrollment follow-up or condition-specific care.
You can manage Care Pathways under the Templates section in Pear Suite.
Key features:
- You can create multiple Care Pathways for different programs.
- One Care Pathway can be set as Default, which means it will automatically begin for every new member added to the platform.
- Each pathway consists of multiple activities scheduled in sequence.
Types of scheduling options for activities within a Care Pathway:
- Immediately: Fires the next activity right away.
- Interval: Allows you to specify a delay (in days, weeks, or months) after the previous step.
- Scheduled: Lets you choose a specific date and time.
NOTE: Condition-Based Triggers - These conditions trigger an activity only if a specific answer choice is selected in a previous activity. This allows you to add decision points to the pathway. This isn't related to the scheduling options listed above, but it works on top of them.
When building a pathway:
- Start with the first activity—it will be the trigger for the Care Pathway.
- Add subsequent activities and set their firing method.
- Link additional steps using logic based on conditions or timing.
Care Pathways help standardize support, reduce manual planning, and ensure members receive the right care at the right time.
