The Care Plan is another tab within the member profile and builds upon the Member Overview section. It provides a structured and exportable document that includes personal health goals, services, and important care information for the member.

Just like Member Overview, the Care Plan pulls information from Data IDs collected during activities. However, it offers more flexibility and options:

• You can create sections that display important Data ID-based information.
• You can include Personal Information such as name, date of birth, and contact details.
• A Medications Section is available to highlight prescribed or ongoing medications.
• Most importantly, the Care Plan allows you to add Goals organized by Domains. Domains are broader categories (like Housing or Nutrition) that help group related goals. Each domain can contain multiple goals and action steps.

To create a Care Plan template:

1. Go to the Organization section from the green sidebar.
2. Select the Care Plan tab from the top bar.
3. Click + Add Section to begin building your care plan layout.
4. Add and label your sections using appropriate Data IDs.
5. Use drag-and-drop to rearrange sections for clarity.
6. To add goals, first create Domains under the General tab in the Organization section. You can then associate goals with those domains.

One of the most useful features of the Care Plan is that it can be exported as a PDF. This is helpful for internal documentation or for submitting care information to a health plan or other agencies.

The Care Plan tab gives teams a structured way to track progress and coordinate care across goals, services, and metrics—all in one place.
