The Care Plan tab within a member profile builds upon the Member Overview to provide a structured, exportable document of personal health goals, services, and critical care information. Organizations can manage multiple templates tailored to specific programs or populations, allowing teams to assign multiple plans to a single member and switch between them as needed.

Step 1 – Access the care plan templates

  1. Navigate to the left-hand green sidebar and select the Templates icon.
  2. Locate and click on the Care Plans tab at the top of the page.
  3. Click the + Add Care Plan Template button to open the builder.

Step 2 – Configure template details and approvals

  1. Enter a Template Title and a brief Description to help your team identify the plan's purpose.
  2. Under the Approvals section, use the checkboxes to decide if you need to Collect Member Acknowledgement or Collect Member Signature.

Step 3 – Define domains and goals

  1. In the Domains & Goals section, select thematic categories like Housing, Nutrition, or Wellness.
  2. Toggle Allow editing of goals and Allow editing of notes to enable staff to make real-time updates directly from the member's profile.
  3. Add specific goal templates or domain-level notes to provide care history and context.

Step 4 – Select member information fields

  1. Navigate to the Member Information section and check the boxes for the demographics you want included (e.g., First Name, DOB, Insurance).
  2. Use the Edit toggle in this section to allow users to update this contact information directly from the care plan without launching a new activity.

Step 5 – Add custom data sections

  1. Click + Add Section to create custom areas, such as an SDOH (Social Determinants of Health) section.
  2. Search for and add specific Data IDs to pull in values collected during previous activities.
  3. Decide for each section whether the data should be Editable or Read-Only for the staff.
  4. Click Create Template to finalize and publish your layout.

Step 6 – Assign and manage care plans

  1. Open a member’s profile and select the Care Plan tab.
  2. Select your template from the dropdown menu to assign it to the member.
  3. Use inline editing (indicated by blue check marks) to update goals, notes, or data values instantly.
  4. Select Export PDF or Collect Member Signature to generate a official summary for health plans or internal documentation.
  5. Click View Version History to track previous answers and changes made over time.
