Member Care Plans
The Care Plan is another tab within the member profile and builds upon the Member Overview section. It provides a structured and exportable document that includes personal health goals, services, and important care information for the member.
Care Plan Templates are created and managed under the Templates section of the platform. This gives organizations the flexibility to create and assign multiple care plan templates based on different programs, populations, or services. Each member can be assigned more than one care plan and teams can switch between them using a dropdown menu.
Each Care Plan Template is built using Data IDs collected through activities. Here’s what can be included:
- Member Information: Add demographics and contact details.
- Medications Section: Highlights any medications the member is taking.
- Custom Sections: Use Data IDs to add sections that reflect specific needs or services.
- Goals & Domains: Goals are organized under Domains, which are thematic categories like Housing, Nutrition, or Wellness. Each domain can contain multiple goals.
- Domain Notes: Add notes directly under each domain to provide context, care history, or updates. These notes follow the same format as other notes in Pear Suite, with redaction and version control.
Care Plan Templates can be configured to define which sections or fields are editable and which are read-only. Users can make changes directly within the Care Plan tab using inline editing, without having to switch views. You can update:
- Goals and their progress
- Notes (domain-level and general)
- Data ID values
- Contact information
All editing actions follow the platform’s permission settings, and the most recent note is shown in the view, while older versions remain available under Member Notes.
Once completed, the Care Plan can be exported as a PDF. This is especially useful for sharing care summaries with health plans or using them for internal documentation.
To build a Care Plan Template:
- Go to the Templates section.
- Click on Care Plans.
- Click + Add Template to create a new care plan layout.
- Add and label your sections using appropriate Data IDs.
- Add Domains (via the General tab), then add goals under each domain.
- Add domain-level notes where helpful.
- Decide which fields should be editable or read-only.
- Save and publish the template.
Care Plans bring together multiple aspects of care—structured goals, important data, medications, and documentation—into one actionable space to support long-term, coordinated care.
