Configuring Provider Review Sign-Off for Activities

Organizations can require provider review and signature for activities by configuring Review Request templates and linking them to activity templates.
This setup ensures that activities are reviewed and locked after provider approval.
Step 1: Create a Review Request Template
1.Go to Templates → Review Requests
2. Click + Add Review Request Template
3. Enter a title and required fields
4. Configure:
- Required fields for requesters and reviewers
- Signature requirement
- Lock activity after sign-off\
5. Save the template
You can also configure actions that trigger when a request is approved.
Step 2: Link the Review Request to an Activity Template
- Go to Templates → Activities
- Open the activity template you want to update
- Find the field Review Request on Completion
- Select the Review Request template
- Click Update Activity Template
Now, every time this activity is completed, a Review Request will be created automatically.
How It Works After Setup
1. Activity Completion Triggers Review Request
- When completing an Activity configured with a Review Request, a Review Request is automatically created on the Member Profile.
Note: If the Review Request does not immediately appear, refresh the page.

2. Submit Review Request for Provider Review

Navigate to the Review Request on the Member Profile
- Note: Your Review Request will be in a “Draft” status
- Open the Review Request
- Complete any required fields
- Click “Save & Submit for Review”
- Review Request status will update to “Sent” status


3. Provider Reviews and Signs
- The provider reviews the request via the Review Request page.

Activity details are easily available via “View Activity Summary”
- Additional information from the Requester may also be available, such as a member’s Care Plan

The provider has several options:
- Reject - This option will automatically close a review. A red “Rejected” status will appear on the Review Request on the Member’s profile.
- Request Additional Information - This option allows Requesters to request additional information from a Requester or end user. An orange “Additional Information Requested” status will appear on the Review Request on the Member’s profile that will require the Requester to update and resubmit the request.
- Sign and approve - This option will prompt the provider to e-sign an Approval of Pear Suite Review for Member Services


4. Activity Becomes Locked
Once the Review Request is signed:
- Activity is automatically locked
- Editing is restricted for all non-billing fields
- The Member Profile will indicate the activities that are locked with a “Locked” status

Go to Review Requests tab on Templates page and click +Add Review Request Template
Create a title, fill in the appropriate fields, and Save Changes
- Admin can require certain fields from Requesters (aka. End users) and Reviewers.
- Enable Review Signature Required and Lock activity after sign-off by checking each checkbox.
- Actions can be triggered on approval of a Review Request
3. Go to the Activities tab on the Templates page and select the activity that requires provider sign-off.
