The Fillable PDF Generator allows organizations to upload standard PDF forms and map their fields to specific Data IDs within Pear Suite. This automation enables the platform to instantly populate assessments, referrals, and consent forms with information already collected from a member’s record, saving time and reducing manual entry errors.
Pre-configuration checklist
Before you begin building your template, ensure you have the following ready: A blank, flattened PDF form (e.g., a state-mandated assessment or local referral form). The specific Data IDs you intend to map (e.g., member_dob, primary_diagnosis). Administrative or "Owner" permissions to edit templates.
Step 1 – Upload and configure the template
- Navigate to the Templates section from the main sidebar.
- Select the PDF Templates tab at the top of the page.
- Click + Add PDF Template and upload your blank file.

Step 2 – Map data fields
- Open the template and highlight the specific area on the PDF where data should appear.
- Choose the appropriate Field Type based on the data format:
- Text: Standard values (Name, ID numbers).
- Date: Formatted date strings (you can customize the display format).
- Multiline Text: Best for free-text notes or long descriptions.
- Checkbox: Automatically places a checkmark based on a specific logic or response.
- Select the corresponding Data ID that should populate that field.
- Adjust the font size or alignment if needed, then click Save Changes.

Step 3 – Generate a completed form
Once your template is saved, it is ready to use within any individual member's record:
- Open a Member Profile and go to the Member Files section.
- Click + File and select the Upload Files / Fill Forms option.
- Choose your saved Fillable PDF Template from the dropdown menu.
- Review the information that automatically pulls in from the profile. You can manually update or fill in any missing fields at this stage.
- Click Save. The completed document is now stored in the member's files for viewing, downloading, or printing.
