Overview

Overview

The Fillable PDF Generator lets organizations upload PDF forms, map fields to Data IDs in the platform, and automatically fill in member information that matches those mapped fields. This feature makes it easy to complete standard forms such as assessments, referrals, and consent forms using data already collected in Pear Suite.

Where This Lives in the Platform

  • Go to Templates from the Main Menu Bar and select the PDF Templates tab.

What You Need Before You Start

  • A blank PDF form.
  • Appropriate Data IDs that can be mapped to the appropriate PDF fields.
  • Access to edit templates as a platform user.

Configure a Fillable PDF Template

  • From the PDF Templates tab, click Add PDF Template.
  • Upload your blank PDF.
  • Highlight the area on the PDF you want to add a fillable field.
  • Choose the Field Type:
    • Text: Use this field type when you want to display the value chosen in from the Data ID.
    • Date: Choose this to input a date from a Data ID. You can customize the date format.
    • Multiline Text: This is best used for sections that have a free-text section. This will create multiple lines of text within the mapped area on the PDF.
    • Checkbox: Use this to create a checkmark in a mapped out box based on an answer response. 
  • Select the Data ID that should fill that field.
  • Adjust formatting options if needed.
  • Click Save Changes.

Once saved, the template appears in PDF Templates and becomes available for use in a member’s profile.

Generate a Fillable PDF in a Member Profile

  • Open the member’s profile and go to Member Files.
  • Click Plus File, then select Upload Files / Fill Forms.
  • Choose a Fillable PDF Template from the dropdown.
  • Review the fields that fill with member data.
  • Add or update any missing information in the form.
  • Click Save to create the completed version.

The completed PDF is automatically stored in Member Files, where it can be viewed, downloaded, or printed.
