How Recommendations Are Generated Reviewing the Recommendations List Opening Pathways From Recommendations Manual Pathway Creation When Needed Maintaining a Clean Recommendations List

Overview

Recommendations are automatically generated from the CHW Visit’s risk screening. They serve as the PCHI-required tool for identifying which needs were flagged during the Visit and which Pathways could be opened to address those needs. CHWs use the Recommendations list immediately after submitting the CHW Visit to decide which Pathways they may want to launch for the member.

How Recommendations Are Generated

During the CHW Visit, the risk screening evaluates each domain using PCHI’s logic. Any domain that meets a risk threshold automatically produces a Recommendation. These Recommendations appear in the left-hand column of the Activities screen as each flow is submitted, because the system logs any positive risk response the moment the CHW completes a flow that contains it.

 
 

Reviewing the Recommendations List

After completing the Visit, the CHW reviews the list of generated Recommendations. Each Recommendation includes the specific Pathway options dictated by PCHI for that risk type, giving the CHW a clear view of which needs could benefit from follow-up.

CHWs can work the Recommendations from two locations:

  • The Activities screen, where Recommendations appear in real time as each flow is submitted.
  • The member’s profile, after the CHW Visit is fully submitted. From the profile, CHWs can still review the Recommendations list, choose Pathways to open, and dismiss Recommendations as they complete them.
 
 

Opening Pathways From Recommendations

The CHW selects the appropriate Pathway(s) from each Recommendation and opens them directly from that list. Once the chosen Pathways are opened, the CHW dismisses the Recommendation so it no longer appears in the queue.

 
 

Manual Pathway Creation When Needed

Not all member needs appear through automated Recommendations. CHWs may manually fire additional Pathways based on context gathered during the Visit or through ongoing engagement with the member.

To open a Pathway outside of a Recommendation, the CHW should:

  1. Scroll to the Care Pathway section of the member’s profile.
  2. Select Add Care Pathway.
  3. Choose the appropriate PCHI Pathway they want to add.

This workflow is used when a Pathway needs to be opened that is not tied to a risk screen or Recommendation.

 
 

Maintaining a Clean Recommendations List

Before beginning a new CHW Visit, the Recommendations list should be empty. Clearing out all previous Recommendations ensures that each new Visit produces a fresh, accurate set of PCHI-driven next steps the CHW could take.

Another reason for maintaining a clean Recommendations list is that the PCHI model requires that any Pathway tied to a risk identified during a CHW Visit be opened at the time the risk is identified. These Pathways should not be held for later, even if they will not be prioritized for the member on the same day.
