When you add a goal to a member’s profile, you’re creating a digital roadmap of support. This feature allows you to break down a larger need into smaller, manageable steps. Within a goal, you can assign specific follow-up tasks and document exactly when help was provided or when significant progress was made.
Determining when to use a goal
Goals are designed for member needs that require ongoing attention. You should create a goal when:
- A member has a need that will take more than one interaction or step to address
- You are providing resources, referrals, or activities that require a follow-up to ensure success
- You want to maintain a structured history of progress for clinical or reporting purposes
Differentiating between goals and one-time tasks
Not every interaction requires a goal. You generally do not need to create a goal for one-time tasks that do not require future tracking. Examples include a single, simple referral that does not need a status check or a brief, informal check-in that doesn't change the member's care plan.
The follow-up rule of thumb
