Adding a Member Activities Member Goals Member Notes Billing for Doula Services

In many states, doula services are covered under Medicaid as a benefit to improve maternal health outcomes, reduce birth disparities, and support birthing individuals before, during, and after pregnancy.

Doulas use Pear Suite to document the care and support they provide to members, coordinate referrals for services, track progress throughout the perinatal journey, and generate Medicaid claims for reimbursable doula services.

Adding a Member

To add a new member (client, patient, customer, recipient) to your Pear Suite account first click the Members page from the main navigation menu on the left.  Next, click the  +Add Member button. 

The only required fields are the ones with a red * next to them - typically First Name, Last Name and Phone Number.  However if you are going be generating Medicaid claims on behalf of this member, the following fields will be required to submit a claims:

First Name
Last Name
Phone Number
Date of Birth
Sex
Insurance company
CIN or Subscriber ID
Address

Once entered, click Update at the bottom which will take you to the member's newly created profile.  Expand the next section for next steps.

 
 

Activities

Activity Types:

  • Prenatal & Postpartum Intake:
    • When working with a new member, start with the Doula Services Intake activity. This activity gathers key information such as due date or delivery date, pregnancy history, and preferred birthing location. It also includes Note Templates to document additional details about their physical health, emotional health, and other important factors.
  • Prenatal & Postpartum Visit: 
    • Use this activity to check in with the member during pregnancy or after birth. You’ll follow up on any known needs and help identify anything new they may want support with.
  • Birth Plan:
    • Use this activity to talk with the member about their preferences for labor, delivery, and immediate postpartum care. You’ll cover things like pain management, who they want present, and any personal wishes they want honored.
  • Birth Summary: 
    • Complete this activity after the baby is born. It helps you collect important details about the birth, like how labor went, any complications, and how the member and baby are doing now.
  • Postpartum Depression Screening: 
    • This activity uses the Edinburgh Postnatal Depression Scale to screen for signs of postpartum depression. You’ll ask a set of standardized questions to help identify if the member may need follow-up support.

Adding an Activity:

1. On the member's profile, locate the Activities section. 
2. Click +Activity
3. Select corresponding activity you would like to complete.
4. If you chose to start the activity immediately, click Create and the activity will begin.
5. If you chose to schedule the activity for later, the activity will appear on the member's Activity section with a green arrow to click when you are ready to start.

Based on the member’s responses, Pear Suite will either suggest a next step such as adding a goal or automatically apply a goal to help address their needs. You’ll learn more about using goals in the next section.

Member Goals

Goals help you track what kind of support a member needs and how you’re working together to make progress.

There are two ways goals can be added to a member’s profile:

Automatically during an activity
As you complete activities, Pear Suite may suggest a goal based on the member’s responses. For example, if a member shares they need help getting baby supplies, the platform may recommend a goal to support that need. Some goals may also be added automatically without a recommendation step. Each goal includes action items to guide your work with the member.

Manually from the Goals section
You can also add a goal yourself. In the Goals section of the member’s profile, click +Goal. You can choose from over 20 templates or create a custom goal by entering a name, description, and action items.

 
 

Member Notes

You can create a note in three places: while completing an activity, in the activity details, or directly in the member’s profile.

Once a note is saved, you can edit or redact it, but the original version is always kept for reference. Notes can’t be permanently deleted.

Note Templates provide a structured format with headings and prompts to help guide what you document.

 
 

Billing for Doula Services

When you click Finish at the end of an activity, the activity details will appear. If the activity is billable, you’ll also see the claim details.

Billable activities include:

  • Intake
  • Prenatal and postpartum visits
  • Birth summary

Non-billable activities include:

  • Birth plan
  • Postpartum depression screening

These non-billable activities can still be included as part of a billable visit.

Before completing the activity:

  • Check the start and end time and update them if needed.
  • Make sure to include the total time spent with the member.
  • You can also add a member note on the right side of the page.
  • Review the insurance, procedure and modifier codes, and place of service.
  • Enter the diagnosis code and then finish the activity.
  • After the claim is created, you can check its status and review any errors.

You've now created your first claim!
