California Compliance Requirements
CHW Certificate/Attestation of Work Experience Upload
Each Community Health Worker who is subcontracting under Pear Suite's network must qualify with one of the following:

CHW Certification
- You must obtain and upload a valid CHW/P/R Certificate that includes a completion date and your name.
- This can be added to your Pear Suite profile by clicking the User Settings icon in the upper right
OR
Work Experience
- You must have at least 2,000 hours of qualifying CHW/P/R work experience (paid or unpaid) within the last 3 years.
- Complete an Attestation of CHW Work Experience and upload it to your Pear Suite profile by clicking the User Settings icon in the upper right and clicking +Create in the Certifications section of your profile. Your Pear Suite Customer Success Manager can supply you with the attestation form.
- CHWs qualifying through Work Experience are only eligible to bill for 18 months. After that time, you must obtain a CHW certification to continue billing.

To Upload, select “User Settings” in the top right hand corner

Then, click +Create in the Certifications section of your profile.

Select the appropriate Work Experience or CHW Certification Pathway

Depending on whether you are uploading the a training certificate or work attestation, name the file:
“CHW Certificate"
or
“CHW Work Experience”
Annual Training
To continue to bill annually, CHWs are required to complete six hours of annual training. One of the six hours is required to be DEI training to meet California requirements.

Existing providers: Must complete annually by December 31, 2025, and by December 31 each calendar year thereafter
How to Complete & Submit Attestation:

- Select the attestation bubble
- Enter in the individual CHWs information
- Select “Individual Provider”
- Input Pear Suite’s NPI number - 1619677762
- Input Pear Suite’s TIN number - 862205424
- Under PPG Affiliation, select “other”
- Enter “Pear Suite” in the text box that appears.
- Take a screen capture/ save page as PDF
- Hit the right arrow to submit
OR Submit an attestation from another plan’s training
Additional Training
CHWs must complete 6 hours of training that fall under CHW Core Competencies, which can include the above DEI training. Trainings can be found by signing up at train.org or looking at webinars hosted by health plans.
The Annual Training Attestation will be used to track annual training completion. Reminders will be sent to complete this attestation quarterly to update on progress.

- Select “Make a Copy”
- Fill out the form
- Go to “User Settings” in Pear Suite

- Select “+Create” Training

- Name the file “Name” Annual Training Attestation
- Set Expiration Date as the End of the Current Year
- Upload File
- Create

Arizona Compliance Requirements
Applying for a CHW License in Arizona
All CHWs who will be billing for services in Arizona must apply for a CHW license through the Arizona Department of Health Services. (Official CHW Application Checklist)
To do this, navigate to:
- Individual Licensing Portal
- Once you verify login, select “Individual Portal”
- Select “Add a new license”
- Select “Community Health Worker”
- Select “New Community Health Worker Application”
- Fill out Required Information
- The CHW Work Attestation provided by Pear Suite may be used to complete the application process for those who are not applying after going through an accredited CHW training program and/or using state reciprocation (a certificate gained in another state)
Downloading and Uploading Arizona CHW License
You will need to download your CHW License from AZ Care Check and Upload it to the Pear Suite Platform.
To do this, navigate to:
- https://azcarecheck.azdhs.gov/s/?programType=Community%20Health%20Workers&licenseType=All
- Type in your name
- Select your name
- Under “Current License” click the text in red
- Click “Download”
To Upload in Pear Suite, log in and:
- Go to “User Settings”

- Then, click +Create in the Certifications section of your profile

- Select “Arizona CHW License”

- Name the file “Name" Arizona CHW License
Arizona CHW License Maintenance
All Arizona CHWs must complete 24 hours of continuing education every 24 months and must renew their license every 24 months. Completion of this training is required prior to renewal. Failure to renew will result in termination of ability to bill in Arizona.
Community Health Workers must upload the Continuing Education Attestation to AZ Care Check to renew their license. Arizona requests that all training certificates are retained by CHWs for auditing purposes.
All users in the Pear Suite Platform will be required to upload a HIPPA Training

How to Complete the Free HIPAA Training and Obtain Your Certificate (via Accountable)
Step 1: Access the Training
Step 2: Create an Account
- Click “Get Started” in the top right corner and follow the prompts to create your free account.
Step 3: Navigate to the Training
- After logging in, select “Training” from the menu on the left-hand side.
Step 4: Begin the Course
- Under “HIPAA Training,” click “Start Training.”
Step 5: Complete the Course
- Go through all sections of the HIPAA training and complete the required quizzes.
Step 6: Access and Submit Proof of Completion
- After completing the final quiz, click “View Certificate” at the bottom right of the screen.

A visible preview of the certificate

Submit the screenshot as proof of completion on the Pear Suite platform.
- This allows verification without requiring payment.

- Name the file “Name” HIPAA Training
- Set the Expiration Date as one year from completion of the training
